All student athletes in grades 7-12 are required to submit athletic participation forms annually.  Please submit all forms (including the medical history/physical form) electronically ONLY through Rank One Sport. Medical history/physical forms are valid for a period of 13 months from the date of evaluation by the healthcare professional to allow for one month's grace to obtain a new physical exam with insurance if desired.
 

Please note: New students will not be able to submit electronic forms until they are issued a student I.D. If a new student has an I.D. number, it is possible that you will not be able to submit forms because you may not be in our athletic database yet. If that is the case, please contact the Athletic Training Dept. at lhathletictraining@libertyhill.txed.net or 512-260-5540.
